New Patient Info & Forms

Our Office is always accepting new patients. We are preferred providers for various dental insurance companies. Please contact us if we can be of assistance or help answer your questions about your dental insurance benefits.

If you are transferring to our office please forward your records and current x-rays to us at least one week prior to your appointment.  You may email your records to info@fvdacl.com  

We are looking forward to meeting you!  Please remember to arrive 15 minutes before your scheduled appointment time so that all necessary paperwork can be completed.

If you are a new patient to our office, below contains our new patient forms that will need to be filled out before you arrive at our office.
